simvastatin
Simvastatin is a low-to-moderate intensity statin that helps lower cholesterol. This helps lower the risk of serious problems like heart attack or stroke. It's approved for adults and certain children 10 years and older. Simvastatin tablets are available as brand-name Zocor and a generic. The liquid suspension form only comes as brand-name FloLipid. The simvastatin dose is taken by mouth once each night. It works best when you also eat heart-healthy foods and stay active. Possible side effects are stomach pain, muscle pain, and headache.
What is Simvastatin?
What is Simvastatin used for?
- High cholesterol in adults
- High triglycerides in adults
- Homozygous familial hypercholesterolemia (HoFH) in adults
- Heterozygous familial hypercholesterolemia (HeFH) in people ages 10 years and older
- Lowering the risk of heart or blood vessel problems (e.g., heart attack, stroke) in adults at high risk
How Simvastatin works
Simvastatin is a statin. The main way it works is by blocking a protein in your liver that makes cholesterol. By blocking this protein, simvastatin affects different types of cholesterol and fats in the body.
- It lessens the amount of "bad" cholesterol in the blood. "Bad" cholesterol is also called low-density lipoprotein or LDL.
- It also lowers the amount of a type of fat in the blood called triglyceride.
- It helps raise the amount of "good" cholesterol a little bit in the body. "Good" cholesterol is also called high-density lipoprotein or HDL.

Pros and cons of Simvastatin
Pros
Cons
- Has the highest risk among the statins to cause muscle pain
- Has some drug interactions, like with certain heart medications and grapefruit
- Can't take if you have certain liver problems
- Not recommended to take while pregnant or breastfeeding
Pharmacist tips for Simvastatin
- Take simvastatin once a day in the evening. Try to stick to the same time each night. This helps keep the medication working consistently. It can also help you remember to take it.
- If you miss a dose of simvastatin, take it as soon as you remember. Don't double up your next dose to make up for the missed dose because it could raise your risk of side effects.
- Take simvastatin tablets with or without food. If it's hurting your stomach, try taking it with food next time to help.
- Swallow simvastatin tablets whole. If you have trouble swallowing it, talk to your prescriber about your options.
- Avoid alcohol or limit how much you drink while taking simvastatin. Alcohol can raise your risk of serious liver problems with this medication. Talk to your prescriber about your drinking habits and how to stay safe while taking simvastatin.
- Avoid grapefruit or grapefruit juice while taking simvastatin. Grapefruit can raise the amount of the medication in your body and raise the risk of side effects.
- Tell your healthcare team about all the medications you take or plan to take (including vitamins and supplements). Simvastatin can interact with many common medications. For example, amiodarone (Pacerone), amlodipine (Norvasc), and colchicine can raise the level of simvastatin in your body. This can put you at higher risk of side effects, like muscle damage.
- Make sure to get all the blood tests your prescriber asks for while taking simvastatin. These tests help check your cholesterol levels to see how well the medication is working. Your prescriber might also check your liver to make sure the medication is still safe for you.
- You shouldn't take simvastatin if you're pregnant. Although still unclear, there's a possible risk of harm to an unborn baby. In most cases, it's still encouraged to stop taking statins while pregnant to be safe. Talk to your prescriber about the risks and benefits and the safest option for you.
- It's not recommended to breastfeed while taking simvastatin. There isn't enough information to know if it's safe. But some other statins can pass into breast milk and might harm a baby.
Tips for FloLipid, the oral liquid form of simvastatin:
- Take FloLipid (simvastatin) on an empty stomach. Studies show that food lowers how much of the liquid medication can get absorbed into your body.
- FloLipid (simvastatin) is available in two strengths: 20 mg/5 mL and 40 mg/5 mL. Make sure you know which strength you have and know how to correctly measure your prescribed dose.
- Shake the bottle of FloLipid (simvastatin) for at least 20 seconds before measuring your dose. Use an oral syringe or a dosing cup to measure out the medication. Don't use a household spoon to do this because it might not be accurate.
- Store FloLipid (simvastatin) at room temperature (68ºF to 77ºF). After you open the bottle, the medication is good for 30 days. Throw it away after 30 days, even if there's some medication left. The remaining medication might not work well anymore.

Risks and warnings for Simvastatin
Simvastatin can cause some serious health issues. This risk may be even higher for certain groups. If this worries you, talk to your doctor or pharmacist about other options.
Severe muscle pain and damage
- Risk factors: Ages 65 years and older | Hypothyroidism that's not well-controlled | Kidney problems | Chinese descent | Taking a higher dose of simvastatin | Taking other medications that can also cause muscle problems | Taking medications that interact with simvastatin
Simvastatin can sometimes cause muscle pain or weakness. The risk is higher if you take other medications that cause muscle damage or interact with simvastatin.
In rare cases, simvastatin can cause a serious muscle problem called rhabdomyolysis. This can break down muscles and lead to kidney damage. Very rarely, statins like simvastatin can also cause immune-mediated necrotizing myopathy (IMNM). This is an autoimmune condition that causes muscle inflammation and muscle weakness. It might last even after you stop taking the statin.
Tell your care team if you have unusual muscle pain, soreness, or weakness. They can check what's going on. But get medical care right away if you have severe muscle pain, dark urine, or extreme tiredness. These can be signs of a serious muscle problem like rhabdomyolysis. Depending on your symptoms, you might need to stop taking simvastatin and get treated for the muscle problem.
Liver problems
- Risk factors: History of liver problems | Drinking large amounts of alcohol
Some people taking simvastatin had higher liver enzyme levels. This can be a sign of liver damage. Most times, these changes happened soon after starting the medication. They're usually temporary and don't cause symptoms. But in rare cases, serious liver problems, including liver failure, have happened.
Before starting simvastatin, let your prescriber know if you have any liver problems. They might also ask for you to get a blood test done to check your liver health. People who have liver problems or high liver enzyme levels shouldn't take this medication.
While taking simvastatin, your prescriber might ask for more blood tests to check your liver. Watch for signs of liver damage and get medical help right away if you notice them. These signs include yellowish skin or eyes, stomach pain or swelling, nausea, vomiting, dark urine, or dark or bloody stools. You might need to stop taking simvastatin to protect your liver.
Higher blood sugar levels
- Risk factors: High blood sugar
Some research suggests that statins like simvastatin might slightly raise blood sugar levels. This can sometimes lead to a higher A1C, which is your average blood sugar over about 3 months. Even so, healthcare professionals believe the benefits of statins are greater than this small risk.
Tell your prescriber if you have diabetes or high blood sugar so they know to follow up with you about this. You might need to check your blood sugar more often. While taking simvastatin, remember to eat healthy, balanced meals and get regular exercise.
Possible risk of harm when taken while pregnant or breastfeeding
If you become pregnant, stop taking simvastatin and tell your prescriber right away. This medication isn't recommended during pregnancy unless the benefits are greater than the risks. Statins might harm an unborn baby. Talk to your prescriber about the safest treatment options for you if you're pregnant or planning to become pregnant.
Breastfeeding is also not recommended while you're taking simvastatin. Some statins can pass into breast milk and might harm your baby. Talk to your prescriber or a lactation expert about safe ways to feed your baby.
