By using this coupon, you confirm that you are not using commercial or government insurance, and are a self-paying patient. You also agree to not attempt to claim or submit any costs associated with the medicine(s) you receive under the program to any person or entity, including to any insurance plan (including Medicare Part D plans), health savings and flexible spending accounts, or any other healthcare reimbursement account. You also attest not to seek true out-of-pocket (TrOOP) credit under the Medicare Part D program for the cost of the medicine(s) you receive under this program.
