Metoprolol succinate extended-release (ER) is a beta blocker medication. Beta blockers are a group of medications that treat heart-related medical conditions (and some others).

Metoprolol ER is available under the brand-names Toprol XL and Kapspargo Sprinkle. They’re FDA approved to treat high blood pressure, angina (chest pain), and heart failure

In this guide, we’ll focus on Toprol XL tablets, which are available as a cheaper generic option. We’ll cover what you need to know about typical Toprol XL dosages and what to do if you miss a dose or accidentally take too much.

What’s the typical Toprol XL dosage for adults?

The typical Toprol XL dosage for adults depends on why you’re taking the medication. The dosages we’ll discuss below are recommended by the manufacturer and approved by the FDA. But your healthcare provider may prescribe a different dose based on individual factors. So make sure to discuss your dose with them if you have any questions.

Regardless of your specific dose, it’s best to take Toprol XL with a meal at the same time each day. Toprol XL shouldn’t be crushed or chewed. You should take it whole. If your tablet has a score line (an indented line), splitting it in half along the line is OK.

Keep in mind: Metoprolol is also available as an immediate-release (IR) product called metoprolol tartrate (Lopressor). It’s easy to confuse metoprolol tartrate and metoprolol succinate. If you’re prescribed one of these medications, be sure to double check your pill bottle to confirm which medication you’re taking.

High blood pressure

A common starting dosage of Toprol XL for high blood pressure is 25 mg to 100 mg once daily. It usually takes about a week to see the effects of any given dose. If your blood pressure is still high, your healthcare provider may increase your dose on a weekly basis until your blood pressure is at the desired goal.

The maximum Toprol XL dosage for high blood pressure is 400 mg per day.

Angina

The typical starting dosage of Toprol XL for angina is 100 mg once daily. Your healthcare provider may increase your dose every week as needed to reach your target heart rate. The maximum dose is usually 400 mg per day.

Heart failure

For heart failure, the starting dose of Toprol XL depends on how severe your heart failure is.

One way to determine the severity of heart failure is using the New York Heart Association (NYHA) Functional Classification system. People with heart failure fit into one of four stages. Stage 1 is the least severe and Stage 4 is the most severe.

If you have NYHA Stage 2 heart failure, your starting Toprol XL dosage may be 25 mg per day. If you have more severe heart failure, like NYHA Stage 3 or 4, your starting dose may be 12.5 mg daily.

After starting Toprol XL for heart failure, your healthcare provider may double the dose every 2 weeks until you reach 200 mg (or lower if you can’t tolerate 200 mg).

What’s the typical Toprol XL dosage for children?

Toprol XL is approved to treat high blood pressure in children ages 6 and older. The dosage for children is based on the child’s weight.

The usual starting dose is 1 mg per kilogram body weight once daily. Regardless of the child’s weight, the starting dose shouldn’t be higher than 50 mg per day.

The dose may be increased if the child’s blood pressure isn’t controlled. The maximum daily dose is usually 2 mg per kilogram body weight or 200 mg once daily, whichever is lower.

Are there any dosage adjustments for medical conditions?

Having certain medical conditions might affect what Toprol XL dosage is best for you. For example, older adults and people with liver problems may need a lower starting dose of Toprol XL. This can help lower the risk of developing side effects, such as tiredness and low blood pressure.

What happens if you miss a dose of Toprol XL?

If you forget to take a dose of Toprol XL, skip the missed dose. Resume your regular dosing schedule the next day at your usually scheduled time. Don’t double your dose to make up for the missed dose.

If you’ve missed multiple doses of Toprol XL in a row, let your healthcare provider know. Missing too many Toprol XL doses can cause your blood pressure to rise.

Can you safely stop taking Toprol XL?

Don’t stop taking Toprol XL (or any heart medication) without talking to your healthcare provider first. Stopping a beta blocker abruptly can cause serious side effects like high blood pressure, chest pain, and a fast heartbeat. More specifically, suddenly stopping beta blockers like Toprol XL can lead to serious consequences, like a heart attack

If your healthcare provider recommends you stop taking Toprol XL, they may gradually lower the dose over a couple weeks to avoid these risks.

What should you do if you take too much Toprol XL?

If you take too much Toprol XL, let your healthcare provider know. Even if you’re not sure how much you took, you should talk to your provider. This is because the risks of taking too much Toprol XL can be dangerous, including severely low heart rate and blood pressure. This can lead to heart failure, loss of consciousness, and shock.

Depending on how much Toprol XL you took, your symptoms, and other individual factors, your healthcare provider may just ask you to monitor your heart rate and blood pressure. Or they may want you to seek emergency care.

How to save on Toprol XL

There are ways to save on Toprol XL, which is available as a generic- and brand-name medication. Anyone with a valid prescription, regardless of insurance status, can use GoodRx to purchase a 30-day supply of Toprol XL at an exclusive cash price as low as $39. You can also save by purchasing generic metoprolol succinate.

The bottom line

Toprol XL is a beta blocker. It’s FDA approved to treat high blood pressure, angina, and heart failure. A typical Toprol XL dosage usually starts at 12.5 mg to 100 mg once daily, depending on what you’re using it for. Then, your healthcare provider may increase your dose over time to get the most benefit from the medication.

If you miss a dose, skip it and resume taking Toprol XL with the next scheduled dose. But don’t stop taking Toprol XL abruptly. This can lead to serious side effects like high blood pressure and a higher heart rate. If you’re interested in stopping Toprol XL, talk to your provider about whether it’s safe, and how to slowly lower your dose to avoid side effects.
