tolvaptan

Generic Jynarque
Used for Kidney Disease

Jynarque (tolvaptan) helps protect kidney function in adults with autosomal dominant polycystic kidney disease (ADPKD) who are at risk of their condition getting worse quickly. It works as a vasopressin-2 (V2) receptor antagonist. Jynarque (tolvaptan) comes as a tablet that's taken by mouth twice a day (8 hours apart). At the start, your dose is slowly raised based on how you respond. Common side effects include feeling thirsty and needing to urinate more often. It can also cause serious liver damage. Because of this risk, it's only available through a special Risk Evaluation and Mitigation Strategy (REMS) program.

How Jynarque (tolvaptan) works

Jynarque (tolvaptan) is a vasopressin V2 receptor antagonist. It blocks a hormone in the body called vasopressin from attaching to the V2 receptor in the kidneys. It helps prevent cysts from growing on the kidneys in people with ADPKD. This can help protect kidney function by slowing kidney damage.

Pharmacist tips for Jynarque (tolvaptan)

pharmacist
  • You can take Jynarque (tolvaptan) with or without food. But avoid grapefruit and grapefruit juice because it can cause your body to absorb more medication. This can raise your risk of side effects.
  • Make sure you know how much Jynarque (tolvaptan) to take and when to take each dose. The blister card has instructions to help you follow the dosing schedule. Usually, you take a higher dose when you wake up and a lower dose about 8 hours later. For example, the typical starting dose is 45 mg when you wake up and 15 mg about 8 hours later.
  • Check the shape of your Jynarque (tolvaptan) tablet before taking it. This helps make sure you have the right dose. The 15 mg tablet is triangle-shaped, the 30 mg tablet is round, the 45 mg tablet is square, the 60 mg tablet is rectangular, and the 90 mg tablet is has five sides.
  • If you miss a dose of Jynarque (tolvaptan), skip it. Wait to take your next dose as usual at the regularly scheduled time. Don't take extra tablets to make up for the missed dose because this can raise your risk of side effects.
  • Swallow Jynarque (tolvaptan) whole. If you have trouble, talk to your prescriber. The tablets don't have a score line (line in the middle of the tablet to cut), making them hard to split evenly. This could lead you to take the wrong dose.
  • Mark your lab appointments on your calendar so you don’t miss them. It's important to get these tests done when your prescriber tells you to during treatment. This helps check your liver and electrolyte levels and make sure Jynarque (tolvaptan) is still safe for you. Talk with your care team about your test results and what they mean for your treatment.
  • Jynarque (tolvaptan) often makes you feel thirsty and urinate out more water. It can also sometimes cause dry mouth. Keep a water bottle nearby and drink water when you feel thirsty and during the day and night. This helps manage side effects and avoid dehydration.
  • If you feel lightheaded or dizzy while taking Jynarque (tolvaptan), drink something hydrating right away. Get medical help right away if you faint, feel confused, or have a fast heartbeat, muscle cramps, or dark urine. These can be signs of serious dehydration.
  • Tell your care team about all medications you take or plan to take. Jynarque (tolvaptan) can interact with many medications. These include some antifungals, such as ketoconazole and fluconazole, and some antibiotics, like clarithromycin. These medications can raise your risk of side effects from Jynarque (tolvaptan). Your prescriber might need to adjust your treatment to help keep you safe.
  • If you're pregnant or thinking of becoming pregnant: Let your prescriber or obstetrician-gynecologist (OB-GYN) know right away. It's not clear how safe Jynarque (tolvaptan) is during pregnancy. Your prescriber or OB-GYN can help you understand the risks and benefits of treatment. They can discuss with you about a safe option for you.
  • If you're planning to breastfeed: It's not recommended to breastfeed while taking Jynarque (tolvaptan). The medication has the potential to cause serious side effects in a breastfed baby. Talk with your prescriber about the risks and benefits. You can also ask your prescriber or a lactation expert and safer feeding options.

Risks and warnings for Jynarque (tolvaptan)

Jynarque (tolvaptan) can cause some serious health issues. This risk may be even higher for certain groups. If this worries you, talk to your doctor or pharmacist about other options.

Liver damage

Black Box Warning Jynarque (tolvaptan) can cause serious liver problems that might lead to liver failure. This can sometimes be life-threatening. Because of this risk, the medication is only available through a special safety program. This program is called the Tolvaptan for ADPKD Shared System REMS.

While taking Jynarque (tolvaptan), you'll need regular blood tests to check your liver. There will be a lab schedule for you to follow. Make sure to keep all of your lab appointments.

Watch for signs and symptoms of liver damage. This includes tiredness, loss of appetite, nausea, vomiting, stomach pain, or dark urine. Other symptoms are fever, rash, itching, and yellowing of the skin or eyes. Stop taking Jynarque (tolvaptan) and call your prescriber right away if you notice any of these symptoms.

If your blood tests show signs of liver damage or if you have symptoms, your prescriber will decide what to do next. You might need to stop taking Jynarque (tolvaptan) for some time. When it's safe to restart, you might need to get your liver checked more often. If the liver damage is serious, you might need to stop taking Jynarque (tolvaptan) completely to help prevent further liver damage.

High sodium levels, dehydration, and low blood volume

Jynarque (tolvaptan) makes your body get rid of more water through your urine. This can put you at risk of dehydration, low blood volume, and high sodium levels.

Before starting Jynarque (tolvaptan), your prescriber will check your sodium levels. During treatment, they'll continue to check your sodium levels, hydration, and blood volume. These must stay in the normal range to keep taking the medication safely.

Be sure to stay hydrated while taking Jynarque (tolvaptan). Drink enough water, especially when you feel thirsty.

Call your prescriber if you feel dizzy, confused, very exhausted, or notice dry, cracked skin. Get medical help right away if you notice weight loss, severe lightheadedness, fainting, muscle cramps, dark urine, or a very fast heart rate. These can be signs of severe dehydration.

Interactions with other medications

Some medications can raise the levels of Jynarque (tolvaptan) in your body. Examples include ketoconazole, itraconazole, and ritonavir. These can put you at higher risk of serious side effects of Jynarque (tolvaptan), like liver problems.

Before starting Jynarque (tolvaptan), tell your care team about all the medications you take. Don't start any new medications without checking with your prescriber first. Your care team can make sure your medications are safe to take together. They might need to adjust your treatment. This might include switching some medications or lowering the dose of others.
