This Program operates outside of any third-party insurance.  Patients enrolled in any state or federally funded healthcare insurance program may not submit any claims for reimbursement for prescriptions purchased as part of this Program. Out-of-pocket expenses incurred when using this Program cannot be applied toward any government insurance benefit out-of-pocket spending calculations, such as Medicare Part D True Out-of-Pocket Costs (TrOOP).  Patients who purchase a prescription under this Program agree that they will not utilize state or federal healthcare insurance coverage for the specified product throughout the entire calendar year.

Patients enrolled in commercial insurance plans may not submit any claims for reimbursement for prescriptions purchased as part of this Program. Subject to applicable state law, patients enrolled in commercial insurance plans should evaluate the terms and conditions of your relevant insurance plan(s), or contact your insurer, to determine whether expenses under this Program would be eligible to count toward your insurance deductible or maximum out-of-pocket costs. Patients enrolled in commercial insurance plans residing in California and Massachusetts may not apply any amounts paid under this Program toward insurance deductibles or maximum out-of-pocket costs.

Any unit subject to the statutory ceiling price under Section 340B of the Public Health Service Act shall not be eligible for this Offer.

This Program does not include all Pfizer products. Program available to residents of the United States and Puerto Rico only. Cannot be combined with any other external savings, free trial, or similar offer for the specified prescription. Data related to your redemption of the coupon may be collected, analyzed, and shared with Pfizer, for market research and other purposes related to assessing Pfizer’s programs.  This Program is not health insurance.  No membership fees.  Data shared with Pfizer will be aggregated and de-identified; it will be combined with data related to other offer redemptions and will not identify you.
