Millions of people in the U.S. get sick with influenza (flu) every year. It’s common for healthcare professionals to prescribe flu antivirals to help shorten the illness. Xofluza (baloxavir marboxil) is a flu treatment that works with only a single dose. What’s more, this antiviral can also help prevent the flu if you were recently exposed to someone who’s sick with it.

With an average retail cost of over $200 per dose, Xofluza may seem inaccessible for many people. But don’t count this treatment out just yet. Below are details on the cost of Xofluza without insurance, how likely it is to be covered by insurance, and ways to save.

How much is Xofluza without insurance?

The cost of Xofluza without insurance depends on the dosage you’re taking. Xofluza dosages are based on body weight. Regardless of which formulation you’ve been prescribed, you only need to take a single dose to treat the flu or prevent the flu after exposure.

The table below lists the average retail costs for the two most common Xofluza doses. Xofluza doesn’t have a generic version available.

Which factors affect the cost of Xofluza?

There are multiple factors that affect the cost of Xofluza, including:

  • Location: Where you live affects the cost of your prescription medications. For instance, people in New Orleans pay more overall for prescription medications than people in Denver.

  • Pharmacy: Different pharmacies may charge different amounts for medications, even in the same neighborhood. You can use GoodRx to compare medication prices at different pharmacies in your area.

  • Dose and formulation: The average cost of Xofluza varies depending on the dose being filled. For example, one 40 mg tablet has an average retail cost of $222.50. The average retail cost of one 80 mg tablet is $223.98.

  • Insurance coverage: If you have insurance that covers Xofluza, you’ll likely pay less for your prescription. But the final cost will also depend on whether you have deductibles, copays, or coinsurance.

Does insurance cover Xofluza?

Most commercial insurance and nearly all Medicaid plans cover Xofluza. But it’s common for Medicaid plans to require prior authorization before they’ll cover it. Less than half of Medicare plans and plans purchased through the Affordable Care Act (ACA) marketplace cover Xofluza.

It’s a good idea to review your plan’s summary of benefits and coverage, as well as its formulary (list of covered medications). These resources can help you determine if your plan covers Xofluza. You can also call the member services number on your prescription insurance card for details.

How different insurance plans cover Xofluza

The tables below show how likely different insurance providers are to cover Xofluza 40 mg and 80 mg tablets. They also show how likely plans are to require prior authorization or step therapy (trying other medications first) before they’ll cover them.

Tips if Xofluza isn’t covered by insurance

If your insurance doesn’t cover Xofluza, try these options:

  • Use Xofluza’s cash pay option to lower the out-of-pocket cost to $50.

  • Contact your prescriber to discuss alternative flu antivirals. Some options, such as Tamiflu (oseltamivir), have lower-cost generic versions available or may be preferred by your insurance provider.

Ways to save on Xofluza

There are ways to save on your Xofluza prescription with and without insurance. Consider these three options, and see how much you could save.

1. Pay as low as $35 with a copay savings card

Xofluza’s manufacturer, Genentech, offers a copay savings card for people with commercial insurance. If you’re eligible, you may pay as little as $35 for Xofluza.

You can also use the copay savings card if you’re uninsured, but it provides a maximum savings of $70. So you’ll likely be able to save more by using Xofluza’s $50 cash pay option (described in more detail below). An advantage of the card is that you can use it at most local pharmacies. The cash pay option can be redeemed only through select online pharmacies.

You can use the Xofluza copay savings card up to two separate times. So don’t discard the information after the first use in case you get the flu twice this season.

You can download the coupon by visiting the manufacturer’s website or by texting “GETCOUPON” to 96260 (message and data rates apply).

To qualify for the Xofluza copay card, you must:

  • Be at least 18 years old, or be the parent or guardian of a child and redeem the coupon on your child’s behalf

  • Have commercial insurance or be uninsured

  • Not be covered by a federally funded or state-funded insurance provider, such as Medicare, Medicaid, or Tricare

  • Be prescribed Xofluza for the flu or flu prevention

  • Fill your Xofluza prescription in the U.S. or a U.S. territory

For more information about the Xofluza copay card, call 1-855-963-5892.

2. Use Xofluza’s $50 cash pay option

If you’re uninsured or your insurance doesn’t cover Xofluza, you may be able to pay $50 for the medication through the manufacturer’s cash pay option. This will likely be more cost effective in this situation than using the copay card discussed above.

Genetech has partnered with two online pharmacies for Xofluza’s cash pay option. You’ll select the pharmacy you’d like to use when you access this savings opportunity. Once the pharmacy processes your prescription, you’ll receive Xofluza in the mail via same-day delivery.

Same-day delivery may not be available in all areas. Check with the Genentech-partnered online pharmacy you’ve selected to make sure they can deliver Xofluza to you.

3. Tap into your HSA, FSA, or HRA

You can pay for Xofluza using a health savings account (HSA), a flexible spending account (FSA), or a health reimbursement arrangement (HRA), if you have one. These accounts let you use pretax dollars for qualifying medical expenses, such as prescription medications. They can help you save money through tax savings.

FSA funds have an expiration date, so be sure to use them before the deadline. HSA dollars roll over, so you can save them for later use. HRA funds may or may not roll over, depending on the plan.

The bottom line

The cost of Xofluza (baloxavir marboxil) without insurance depends on your dosage. On average, one 40 mg tablet costs about $222.50 without insurance. The average retail price of one 80 mg tablet is $223.98.

Most commercial insurance plans and almost all Medicaid plans cover Xofluza. But Medicaid plans often require prior authorization first. Less than half of Medicare plans and plans purchased through the Affordable Care Act (ACA) marketplace cover Xofluza.

You may be able to save on Xofluza with a manufacturer copay savings card. If you have commercial insurance, you may pay as little as $35 for the medication with this card. If you’re uninsured or your insurance doesn’t cover the medication, you may be able to pay $50 for Xofluza using the manufacturer’s online cash pay option.

Methodology

We obtained national prescription coverage data for each medication from Managed Markets Insight & Technology (MMIT). The data reflects health insurance coverage as of December 2025.

We calculated the percentage of enrollees in plans that cover each medication by dividing the number of enrollees covered for the medication within a specific insurance channel (e.g., ACA, Medicare, Medicaid) by the total number of enrollees in all plans within that channel.

We determined the percentage of enrollees in plans with a prior authorization requirement by calculating the proportion of enrollees in plans that required prior authorization for the medication within each channel.

Finally, we measured the percentage of enrollees in plans with a step therapy requirement by calculating the proportion of enrollees in plans that imposed step therapy requirements for the medication in each channel.

These estimates, derived from national MMIT coverage data, provide insight into health insurance coverage for each medication. They take into account restrictions such as prior authorization, step therapy, and quantity limits. The percentages are expressed relative to the total number of enrollees covered by various insurance channels, including ACA plans, commercial (excluding ACA) plans, Medicare, and Medicaid (both state-run and managed care plans).
