Save on related medications

Stiolto Respimat (tiotropium / olodaterol) is one of several inhalers that can help manage chronic obstructive pulmonary disease (COPD). This combination inhaler can relax your airways and help you breathe better. Siolto Respimat is a maintenance inhaler that you use once a day to help reduce symptoms and prevent flare-ups.

If you’re just starting Stiolto Respimat, cost may be top of mind. And if you’re looking for ways to save, you’ve come to the right place. Below are details on how much Stiolto Respimat costs without insurance — plus savings tips to consider.

How much does Stiolto Respimat cost without insurance?

The typical dosage for Stiolto Respimat is 2 puffs inhaled once a day. Each inhaler contains 60 puffs or inhalations (a 1-month supply). The average cost of Stiolto Respimat without insurance is $640.50 per inhaler. And the average price for 3 inhalers (a 3-month supply) is $1919.92.

Stiolto Respimat is a brand-name medication. A generic version likely won’t be available until at least 2027.

What factors affect the cost of Stiolto Respimat?

There are several factors that affect the cost of Stiolto Respimat, such as:

  • Location: Where you live can impact how much you pay for medications. For example, people living in Lexington, Kentucky, tend to pay more overall for prescriptions than people in Atlanta.

  • Pharmacy: Pharmacies may charge different prices for medications (even within the same area). It’s a good idea to use GoodRx to compare prices before filling your Stiolto Respimat prescription.

  • Quantity: The number of inhalers you fill at one time also affects how much you pay.

  • Insurance and discounts: Using insurance (if you have it) or applying discounts can help you lower your prescription costs.

Does insurance cover Stiolto Respimat?

Most insurance plans cover Stiolto Respimat. But there are instances where plans require prior authorization. This is when your prescriber needs to give your insurance provider information about why you need a medication. And while uncommon, some plans may require you to complete step therapy (trying other medications first) before they’ll cover Stiolto Respimat.

Nearly all Medicaid plans cover Stiolto Respimat. But slightly more than 25% of plans will require a prior authorization for it. And about 10% will ask you to complete step therapy.

Over 90% of commercial plans cover Stiolto Respimat. And nearly 85% of Affordable Care Act (ACA) marketplace plans cover it. It’s rare for these plans to require prior authorization or step therapy.

Medicare coverage is slightly more limited, with a little over 60% of plans covering Stiolto Respimat. But these plans don’t typically require prior authorization or step therapy.

It’s a good idea to check your insurance provider’s formulary (list of covered medications) and summary of benefits and coverage. This can help you determine if your plan covers Stiolto Respimat and what the cost might be. If you’re unsure, call the member services number on your prescription insurance card for more details.

What to do if Stiolto Respimat isn’t covered by your insurance

If your insurance doesn’t cover Stiolto Respimat, try these options:

  • See if you qualify for Stiolto Respimat’s manufacturer copay savings card or patient assistance program (more details below).

  • Request a formulary exception from your insurance company. If your request is denied, you have the right to appeal the decision to seek coverage.

  • Ask your prescriber about alternatives. There are other COPD inhalers available, some of which have generic options that may cost less.

  • When it’s time for open enrollment, compare different health insurance plans to find one that provides better coverage for Stiolto Respimat.

Ways to save on Stiolto Respimat

Here are four ways to save on Stiolto Respimat, regardless of insurance status.

1. See if you qualify for a copay savings card

If you have commercial insurance or are uninsured, you may pay as little as $35 per month for Stiolto Respimat with a manufacturer copay savings card.

To be eligible for the Stiolto Respimat copay savings card, you must:

  • Have commercial insurance or be uninsured

  • Not have government-funded insurance, such as Medicare, Medicaid, or Tricare

  • Not be receiving medication or reimbursement for Stiolto Respimat through a patient assistance program

  • Be at least 18 years old

  • Have a valid prescription for Stiolto Respimat for an FDA-approved use

  • Live in the U.S. or a U.S. territory

You can sign up for the Stiolto Respimat copay savings card online. Your prescriber or pharmacist can also help you sign up. If you have questions or need help activating your card, call 1-877-264-2440.

2. Check if you’re eligible for a patient assistance program

If you’re uninsured or underinsured, you may qualify for Stiolto Respimat’s patient assistance program. This program offers the medication free of charge for people who meet certain financial criteria.

To qualify for Stiolto Respimat’s patient assistance program, you must:

  • Be uninsured or have Medicare Part D

  • Live in the U.S. or a U.S. territory

  • Receive your Stiolto Respimat prescription from a U.S.-licensed prescriber

  • Meet certain financial requirements and be able to show proof of income

Your prescriber can help you apply for the Stiolto Respimat patient assistance program online. If you have access to a printer, you can also download and print the form to give to your prescriber. For questions about the program, call 1-800-556-8317.

There are also independent patient assistance programs available through nonprofit organizations. These may also be able to help you save on Stiolto Respimat. Each program has its own eligibility criteria. If a program isn’t accepting new applicants, you may be able to join a waitlist to be notified when enrollment is open again.

Examples of independent patient assistance programs include:

3. Ask about 90-day supplies

Some pharmacies and insurance plans offer discounts if you fill a 3-month supply of your medication. If available, this could help lower your monthly cost for Stiolto Respimat over time. What’s more, it can help reduce the number of trips you make to the pharmacy.

Some insurance plans also offer lower copays or coinsurance if you use their preferred mail-order pharmacies. Check with your insurance provider to see if this option is available for you.

4. Tap into your HSA, FSA, or HRA

Consider using a health savings account (HSA), flexible spending account (FSA), or health reimbursement arrangement (HRA) to purchase Stiolto Respimat. These accounts let you use pretax dollars to pay for prescription copays and other qualified medical expenses. This can help you save money through tax savings.

Just be sure to review your account’s details regarding expiration dates. FSA dollars have an expiration date. But HSA funds roll over and can be saved for later. HRAs may or may not expire, depending on your account.

Methodology

We obtained national prescription coverage data for each medication from Managed Markets Insight & Technology (MMIT). The data reflects health insurance coverage as of January 2026.

We calculated the percentage of enrollees in plans that cover each medication by dividing the number of enrollees covered for the medication within a specific insurance channel (e.g., ACA, Medicare, Medicaid) by the total number of enrollees in all plans within that channel.

We determined the percentage of enrollees in plans with a prior authorization requirement by calculating the proportion of enrollees in plans that required prior authorization for the medication within each channel.

Finally, we measured the percentage of enrollees in plans with a step therapy requirement by calculating the proportion of enrollees in plans that imposed step therapy requirements for the medication in each channel.

These estimates, derived from national MMIT coverage data, provide insight into health insurance coverage for each medication. They take into account restrictions such as prior authorization, step therapy, and quantity limits. The percentages are expressed relative to the total number of enrollees covered by various insurance channels, including ACA plans, commercial (excluding ACA) plans, Medicare, and Medicaid (both state-run and managed care plans).
