“GLP-1 medications work in the body by reducing your appetite. They help regulate your blood sugar, and they also help control cravings,” says Stacia Woodcock, PharmD, Pharmacy Editor at GoodRx.
Compounded GLP-1 injections were available for a couple years due to shortages of semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound). Often, these compounded versions cost less than their branded counterparts.
A compounded GLP-1 medication is a custom-made version of a branded GLP-1 medication. As a compounded medication, it’s not approved by the FDA. Instead, it’s made from scratch in a compounding pharmacy. It contains the same active ingredient (like semaglutide or tirzepatide), but it may vary in dosage, ingredients, quality, and method of delivery.
Brand-name GLP-1 medications have been approved by the FDA. Before a new medication is approved and available for use, it must undergo years of research and development, including clinical trials. There is also a fee to apply for FDA approval. A 2024 study found that this process costs an average of $172.7 million for new drugs developed from 2000 to 2018.
Before approval, the FDA inspects the manufacturing facility to ensure it follows strict federal standards, which helps ensure each dose is safe, clean, and consistent, and that all processes are properly documented and controlled.
After approval, the facility must continue meeting these standards through FDA inspections. These added safeguards and quality controls also contribute to higher costs compared to compounded drugs.
The lower cost of compounded GLP-1s may not be beneficial in the long run.
“There are some general guidelines when it comes to how compounded medications are made,” says Woodcock. However, she explains that the individual medications themselves aren't subject to the same FDA review and approval process as branded GLP-1s.
The FDA has received reports of medical issues from compounded GLP-1s, often related to dosing errors.
Compounded GLP-1s can be very helpful during a shortage, or if somebody has an allergy to an ingredient in the branded version. However, they aren’t usually meant to be a long-term substitute for an FDA-approved medication for the general population.
If the cost of a branded GLP-1 is a barrier for you, there are many ways to save. Those savings opportunities aren’t available for the compounded GLP-1s, according to Woodcock.
