Firdapse (amifampridine) is the first FDA-approved medication to treat Lambert-Eaton myasthenic syndrome (LEMS), a rare, autoimmune condition that can cause muscle weakness. The medication can cause seizures as well as many other side effects.
Firdapse (amifampridine) is a potassium channel blocker. It's not exactly known how this medication works to treat LEMS, but it's thought to target your muscles' nerve junctions to help with nerve signaling.
The following side effects may get better over time as your body gets used to the medication. Let your healthcare provider know immediately if you continue to experience these symptoms or if they worsen over time.
Contact your healthcare provider immediately if you experience any of the following.
Side effects that you should report to your care team as soon as possible:
Side effects that usually do not require medical attention (report these to your care team if they continue or are bothersome):
Lambert-Eaton myasthenic syndrome (LEMS) is an autoimmune condition. It's when your immune system attacks the connection between your nerves and your muscles. When this happens, your nerves can't send signals to your muscles. This causes symptoms of LEMS, including muscle weakness and fatigue in the arm and leg muscles, which can make exercising or walking difficult. It's even possible for the condition to affect your muscles that help you swallow and talk.
Your body's ability to break down Firdapse (amifampridine) well depends on your genetics. So your provider might instruct you to do genetic testing before you start this medication. Ask your provider to review your test results with you. If the results show that you aren't able to break down Firdapse (amifampridine) well, your body can't get rid of the medication efficiently. This means that the medication can build up in your body when you take it, which can raise your risk of side effects. To prevent this from happening, you'll need to start Firdapse (amifampridine) at a lower dose.
There haven't been any studies looking at how Firdapse (amifampridine) affects pregnant females, so it's not known if it's safe for your unborn baby. In some animal studies, Firdapse (amifampridine) caused harm to babies born to mothers who received the medication. Because of this potential risk, Firdapse (amifampridine) should only be used in pregnant females if it's clearly needed. If you're pregnant or thinking of becoming pregnant, talk with your provider about the risks and benefits of using this medication during pregnancy. If you're pregnant and are prescribed Firdapse (amifampridine), it's recommended that you enroll in the Firdapse Pregnancy Registry. This registry monitors your and your baby's health outcomes. Contact the Firdapse registry by calling 1-855-212-5856 or visiting the registry website.
No, Ruzurgi is no longer available. Ruzurgi was a brand of amifampridine for children 6 to 16 years old. The FDA's approval for Ruzurgi (amifampridine) is no longer valid. In 2022, a court ruled that Ruzurgi is too similar to Firdapse according to the Orphan Drug Act.
Firdapse (amifampridine) can cause some serious health issues. This risk may be even higher for certain groups. If this worries you, talk to your doctor or pharmacist about other options.
Firdapse (amifampridine) can cause seizures. They can happen in people taking this medication at recommended doses who haven't had a seizure before and can happen at any time during treatment. But your risk is higher if you have medical conditions or take medications that raise your risk for seizures. If you have a seizure or any symptoms of a seizure while taking Firdapse (amifampridine), contact your provider.
The typical starting daily dose is 15 mg to 30 mg by mouth per day, divided into 3 to 4 doses throughout the day. The maximum single dose is 20 mg (or 2 tablets), and the maximum total daily dose is 80 mg (or 8 tablets).
Your provider will change your dose based on if you're having side effects and if it's helping your condition. Your dose might differ if you have kidney or liver problems. It might also differ if your body is unable to break down the medication well (based on genetic testing).
Contraindications are specific health conditions or situations in which a person should not take a medication due to safety concerns. If you have any of the following conditions or if any of the following apply to you, let your healthcare provider know because Firdapse (amifampridine) will not be safe for you to take.
