Ebglyss (lebrikizumab-lbkz) is an injectable biologic medication that’s FDA-approved to treat moderate-to-severe atopic dermatitis (eczema) in people ages 12 and older who weigh at least 40 kg (88 lbs). It’s an interleukin antagonist that targets inflammation that contributes to eczema. It’s an option to consider when topical treatments are not effective or suitable.
Ebglyss comes in both a prefilled syringe and an injection pen. Currently, there’s no biosimilar available for Ebglyss. Depending on many factors, out-of-pocket costs can reach several thousand dollars for an Ebglyss syringe or pen. But you may be able to reduce your costs with a GoodRx discount or one of the manufacturer’s financial assistance programs.
How much does Ebglyss cost?
The manufacturer’s list price for one Ebglyss device is $3,675 as of January 1, 2026. What you pay may differ depending on the pharmacy, its wholesale supplier, and other factors.
Without insurance or discounts, a carton containing 1 prefilled pen of Ebglyss 250 mg/2 mL costs around $4738.40. The average retail price for a carton containing 1 syringe of 250 mg/2 mL is $4176.56
Several factors can affect your cost for Ebglyss. These include:
Pharmacy: Ebglyss is usually dispensed through specialty pharmacies, which may have different pricing and negotiated rates. Your cost can also depend on whether the pharmacy is in your insurance plan’s network.
Where you live: Medication prices vary by location. For example, GoodRx research shows that people living in Little Rock, Arkansas, often pay higher prescription costs, while those in Denver tend to see lower prices.
Dosage: Your cost can vary depending on where you are in your treatment. Initial, biweekly, and maintenance dosages of Ebglyss may be taken on different schedules, such as every 2 weeks versus every 4 weeks.
Insurance: Deductibles, copays, or coinsurance may apply even if your insurance covers the medication.
Discounts: You may be able to lower the cost of Ebglyss if you qualify for one of the manufacturer’s financial assistance programs.
Does insurance cover Ebglyss?
Most insurance types cover Ebglyss, though coverage rates vary significantly. Nearly all Medicaid and commercial plan enrollees have access to the medication, while about 8 in 10 people with Affordable Care Act (ACA) marketplace plans and 3 in 10 Medicare enrollees have access to the medication.
However, a prior authorization requirement (advance insurance approval for coverage) is common, affecting about 8 in 10 people with Medicaid, ACA, and commercial coverage. Step therapy rules (trying other medications first) are also common, affecting about two-thirds of ACA enrollees and more than 8 in 10 people with commercial plans.
To understand your exact costs and coverage requirements for Ebglyss, check your plan’s formulary (list of covered medications) or contact your insurer directly.

Consider these options if your insurance doesn’t cover Ebglyss or your out-of-pocket costs are too high:
See if you qualify for financial assistance through the manufacturer’s copay savings card or the drugmaker’s patient assistance program (see below for more details).
Ask your prescriber about other biologic medications for eczema that may be preferred by your plan and could cost less out of pocket.
Request a formulary exception from your insurer. Your prescriber may need to submit paperwork and a letter of medical necessity explaining why you need Ebglyss.
If your insurer denies the formulary exception request, you can file an appeal.
During open enrollment for health insurance, look for a plan that offers better coverage for the medications you need.
Ways to save on Ebglyss
Paying for Ebglyss out of pocket can mean spending several thousand dollars for one prefilled pen or syringe. But here are six ways to reduce your costs:
1. Apply for the manufacturer’s savings card
Depending on your insurance coverage, you can use the manufacturer’s Ebglyss Savings Card Program to reduce your costs for the medication.
If your commercial insurance covers Ebglyss, you may pay as little as $5 for a 1-month supply (up to 4 pens or syringes). This program has a maximum annual savings of $10,600 and is limited to 14 prescription fills per calendar year. The program also has a lifetime maximum of 24 fills.
If your commercial insurance doesn’t cover Ebglyss, you may pay as little as $25 for a 1-month supply. This card also allows up to 14 prescription fills per calendar year. You must submit a prior authorization and receive a coverage denial before your first prescription fill. To maintain eligibility, you’ll need to submit a new prior authorization with a denial outcome each year.
To be eligible for one of the Ebglyss copay savings card offers, you must:
Have commercial insurance
Have a valid prescription for Ebglyss
Be 18 years or older
Live in the U.S. or Puerto Rico
Not be enrolled in any state or federal insurance program, including Medicare Part D, Medicare Advantage, Medigap, Medicaid, Department of Defense, Veterans Affairs (VA), or Tricare
2. See if you qualify for a patient assistance program
The Lilly Cares Patient Assistance Program may provide Ebglyss at no cost if you qualify. This program is designed for people who are uninsured or have limited insurance coverage, including certain Medicare situations.
To qualify, you must:
Have a valid prescription for Ebglyss
Be a permanent resident of the U.S., Puerto Rico, or the U.S. Virgin Islands
Meet the program’s household income requirements
Not be enrolled in Medicaid, full Low-Income Subsidy (Extra Help), or VA benefits
Your insurance status must also meet one of the following conditions:
You have no insurance
You have Medicare Part D
You have Medicare Part B without any supplemental or secondary insurance (such as Medigap or Medicare Advantage)
You’re not eligible for this program if your insurance plan requires you to apply to a patient assistance program as a condition of coverage (often called an alternate or third-party funding program).
To apply, you’ll need to complete a Lilly Cares application. Your prescriber will also need to complete a section of the application and submit it via fax to Lilly Cares.
You may also be able to get help through an independent PAP, such as: The Patient Access Network (PAN) Foundation, which offers grants to pay treatment costs for people who have a demonstrated need for financial support. The PAN Foundation has an atopic dermatitis fund, which can help with Ebglyss costs. Once approved, you may also qualify for a transportation grant. As of publication time, the fund is closed, but you can join the waiting list.
3. Use a GoodRx discount
If you don’t qualify for either of the manufacturer’s programs, a free GoodRx discount may help reduce your out-of-pocket costs for Ebglyss at some pharmacies.
With a GoodRx discount, you may pay as little as $3713.45 for a carton containing 1 prefilled pen of Ebglyss 250 mg/2 mL or as little as $3713.45 for 1 syringe of Ebglyss 250 mg/2 mL.
Here’s how to get a GoodRx discount for Ebglyss:
Go to www.goodrx.com or download the app.
Enter “Ebglyss” in the search bar.
Add your ZIP code to compare prices at nearby pharmacies.
Choose how you want to access the coupon: text, email, or print. If you’re using the app, you can keep the coupon ready on your phone screen or save it to your digital wallet.
Let the pharmacy staff know you’re using a GoodRx discount before they fill your prescription to prevent any delays.
4. Compare prices at different pharmacies
Although some retail pharmacies may carry Ebglyss, it’s usually filled through a specialty pharmacy. Before filling your prescription, check which specialty pharmacies are in your insurance’s network. Using one of your plan’s preferred pharmacies can help you avoid higher out-of-pocket costs from going out of network.
You can also use the GoodRx website or app to quickly compare prices at local pharmacies. In many cases, simply getting your prescription filled at a different pharmacy can help you save money.
5. Ask for a 90-day supply
Talk to your prescriber about getting a 90-day supply of Ebglyss. Many pharmacies offer discounts for larger supplies if you’re paying entirely out of pocket.
Insurance plans may also reduce copays when you fill a 90-day supply instead of getting monthly refills. If Ebglyss is covered by your plan, find out if your insurer’s mail-order pharmacy offers discounts for a 90-day supply. Savings aren’t guaranteed, but looking into this option could lower your costs.
6. Tap into your HSA or FSA funds
If you have a health savings account (HSA), flexible spending account (FSA), or health reimbursement arrangement (HRA), you can use those funds to pay for Ebglyss and other qualifying medical expenses.
You must use the money in your FSA by the end of the plan year, or you’ll lose it, unless your employer offers a grace period or carryover option. HSA funds, however, roll over annually and are available for future healthcare expenses. HRA rules depend on your employer’s plan design.
The bottom line
A carton containing one prefilled pen of Ebglyss (lebrikizumab-lbkz) 250 mg/2 mL costs an average of $4738.40 without insurance or discount programs. A carton of one syringe of Ebglyss 250 mg/2 mL costs $4176.56 on average. Your location, the pharmacy you choose, and several other factors can affect what you actually pay.
The manufacturer’s patient assistance program may provide Ebglyss at no cost if you’re uninsured or have limited coverage and meet income requirements. If you have commercial insurance, you may pay as little as $5 for a 1-month supply of the medication if you qualify.
You could also lower your out-of-pocket costs for Ebglyss by using a free GoodRx discount, comparing prices at different pharmacies, or paying with a tax-advantaged account.

Methodology
We obtained national prescription coverage data for each medication from Managed Markets Insight & Technology (MMIT). The data reflects health insurance coverage as of January 2026.
We calculated the percentage of enrollees in plans that cover each medication by dividing the number of enrollees covered for the medication within a specific insurance channel (e.g., ACA, Medicare, Medicaid) by the total number of enrollees in all plans within that channel.
We determined the percentage of enrollees in plans with a prior authorization requirement by calculating the proportion of enrollees in plans that required prior authorization for the medication within each channel.
Finally, we measured the percentage of enrollees in plans with a step therapy requirement by calculating the proportion of enrollees in plans that imposed step therapy requirements for the medication in each channel.
These estimates, derived from national MMIT coverage data, provide insight into health insurance coverage for each medication. They take into account restrictions such as prior authorization, step therapy, and quantity limits. The percentages are expressed relative to the total number of enrollees covered by various insurance channels, including ACA plans, commercial (excluding ACA) plans, Medicare, and Medicaid (both state-run and managed care plans).
